FORMS

This is a four part application which includes:

  1. An application form to be filled out by the applicant
  2. A reference form to be filled out by the applicant's advisor
  3. A reference form to be filled out by a University affiliated person of the applicant's choice
  4. A program of study statement to be filled out by the applicant

Application Deadline: February 15, 2012

This form must be completed, signed, and returned by:

  • December 2, 2011 - For Spring 2012 Exchanges
  • April 27, 2012 - For Fall 2012 Exchanges
This form must be completed by all students planning to study in Puerto Rico, where the language of instruction is Spanish.
This is a brief survey concerning your National Student Exchange experience. Your care, patience, and thoughtfulness in completing this form can aid our office in making decisions about improvements in program administration, in advising students who apply for exchange, and in identifying ways to better serve students who will participate in NSE in the future. Thank you.
