Approval is required prior to the employee performing additional duties. This form is for internal department use only and once completed, should be kept in the departmental personnel file for the employee.
In an effort to make our Forms site easier to use, we have updated how the forms are categorized. They are now listed two ways, both alphabetically and by type. Our forms have a brand new look too. The first thing you will notice is the new HR logo on all the forms that are “owned” by Human Resources. And we’ve also added a phone number on the bottom of each form in case you have any questions on how to complete the form and a “revised” date so you can be sure you’re using the latest version.
The best news is that most of our forms are now “form fill-able” and you’ll be able to save them. You’ll need to have at least version 8 of Adobe Reader in order to access and use these features. If you are running an earlier version of Adobe, you’ll still be able to open and print the forms. Check with your departmental LanTech or call the university help desk (515-4357) if you need to upgrade.
Something else you’ll notice on the forms pages is a button titled cross walk. This is a listing of the forms whose names have changed or if two forms have been combined into one.
