Exit Interview Program
The NC State Exit Program is designed to collect climate and other qualitative information from exiting employees to identify work environment issues that contribute to both retention and turnover; to provide exiting employees an opportunity to voice concerns and make suggestions, and to ensure the retrieval of University assets when an employee separates from employment
Who's Covered?
All full-time and part-time SPA and EPA employees are covered under the program at the time they separate from the University.
Employees transferring within the University are not asked to complete the University Exit Questionnaire, although individual departments may choose to implement the departmental questionnaire and interview with transferring employees.
What Is the Policy?
All covered employees should complete a University Exit Interview Questionnaire, complete a Separation Checklist, as well as comply with any departmental exit requirements, such as completing a departmental exit questionnaire and/or interview. Completion of the University Exit Questionnaire form by employees is highly encouraged, but not mandatory.
Exiting employees are strongly encouraged to schedule an Exit Interview.
Employees who would like to participate in an Exit Interview with Human Resources, may contact their Department personnel representatives or Employee Relations, 515-6575 to schedule an Exit Interview. Faculty may contact the Office for Equal Opportunity, 513-2099, to schedule an Exit Interview.
Departments are responsible for the following.
- Distributing the University Exit Questionnaire to each separating employee.
- Scheduling a personal interview for departing SPA and/or EPA professional employees with a representative from Human Resources or OEO for EPA faculty for those employee interested in participating.
- Collecting exit data at the departmental level upon separation of any covered employee. This information can be obtained by distributing a departmental exit interview questionnaire and scheduling a departmental exit interview.
- Ensuring collection of assets from the employee and completion of the university separation checklist
- Mailing a University Exit Interview to all off-campus employees for him/her to complete and return to the designated office.
Where Is the Questionnaire?
All employees leaving NC State, including faculty, EPA, and SPA, can fill out the Exit Interview Questionnaire by completing the form ONLINE or by printing out a hard copy and returning the completed form to Human Resources, Campus Box 7210 for SPA and EPA professionals; or the Office of Equal Opportunity, Campus Box 7530, for EPA Faculty.
What Happens to the Information Presented in the Questionnaire or During the Interview?
If information received suggests a violation of university policy or similar critical concerns, Human Resources or the OEO will immediately follow up with the appropriate department.
In the month following the employee's separation, copies of the completed Exit Questionnaire are distributed to the appropriate Vice Chancellor or Dean (or designee) and Human Resources directors (for SPA and EPA professionals), who may forward this information to department heads and supervisors as appropriate.
Human Resources and the Office for Equal Opportunity produce an annual report of exit information and forward the report to University administrators.
