Name and/or Address Changes
Most NC State benefit plan administrators will accept name and/or address changes through this one-stop, easy process. Supplemental savings plan and retirement systems require a signature. Contact information for specific plan administrators is provided on the address change page.
HR System and Payroll
You can update your address in the Human Resources/Payroll System through MyPack Portal. After signing in, click on "Employee Self Service", and then click on “Personal Information”. Here you can change your home address, your email address and your home or cell phone numbers. If you are unable to access MyPack Portal you can complete a change of address form. Please follow the directions on this form for submission. For a name change, you must bring your social security card showing your new name to your departments Personnel/Payroll Representative.
Social Security Card
If you change your name, you will need to get a new Social Security Card from the Social Security Administration. Your Social Security number will stay the same and a new card will be issued showing your updated name. Complete the Social Security Card (SS-5) Application and submit proof of your identity as instructed. Visit the Social Security Administration website for more information.

Non-Citizen Requirements
ALL NON- US CITIZENS must report any address changes to US Citizenship and Immigration Services (USCIS) while residing in the United States, within 10 days of the change. Non-citizens should notify USCIS if no longer living at the address listed on the I-94 form upon entry into the United States by submitting Form AR-11 (Alien's Change of Address Card). Each family member must submit an individual form. The AR-11 form may also be ordered by phone at 1-800-870-3676. The NC State International Employment Office offers assistance to non-citizens at NC State and must be notified of your address change.
Taxes
A move or name change due to marriage or divorce may lead to the need for updating your Federal and State tax withholding exemptions.You can review and change your tax withholding online, using your Unity ID.
