Meet with plan 403(b) representative or contact vendor’s
customer service department get questions answered about
their product.
Select a vendor(s) complete the appropriate vendor application
kit along with a University
Salary Reduction Form. Return 403(b) plan application
form to the vendor(s) and the Salary Reduction form to the
Benefits office (Campus 7215). You can contribute to more
than one 403(b) plan as long as you do not exceed the annual
403(b) contribution limit from all plans combine.
When completing the Salary Reduction form be sure to
indicate the first date of the month you would like for
your contributions to begin and return this form by the
10th of the month in which you want the payroll deductions
to begin. The plan representative may return this form for
you.
