TriCare Supplement Insurance printable version

The TRICARE Supplement plan is offered to State of North Carolina TRICARE eligible employees as an alternative choice to the State Health Plan (SHP). If you are eligible for the TRICARE medical plan provided by the Department of Defense, you have the opportunity to enroll yourself and your eligible dependents in the ASI TRICARE Supplement plan. Eligible employees must drop their SHP coverage to enroll in the TRICARE Supplement plan.

What is the TRICARE Supplement Plan? The TRICARE Supplement plan provides TRICARE beneficiaries additional coverage that pays 100 percent of the member’s out-of-pocket covered costs. The State of North Carolina pays for coverage under the Plan for the Employee only premium. You may enroll your spouse or dependents by paying the cost as illustrated below.

How do I enroll? Please complete the TRICARE enrollment form, complete the SHP enrollment form to cancel your enrollment and return these two forms with a copy of your Military ID Card to the Benefits Office.

You are also required to complete a TRICARE Other Health Insurance (OHI) form if you or your dependents were previously enrolled under the SHP or another health plan. Send completed OHI form to TRICARE. The OHI form is required to notify TRICARE that they will become primary payer. Failure to notify TRICARE may result in claims being denied.
